QuarkXPress Document and PHP Script for eSeminar "Forms"

Hi, all,

thank you very much for the positive feedback to our eSeminar about forms.

As many people have asked for the QuarkXPress document and the PHP script, here they are to download:

As mentioned in the eSeminar the data verification are far from complete, you still need to do checks for lastname and country and also make sure that the field are being hidden again when the form is resubmitted. And careful with the PHP script, as it is a mailer.
